How to Register:

If you are moving to or live within our practice area and would like to register with us, please bring along your NHS medical card or write to our practice manager. We will not refuse to accept any patient on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition; although on occasions when our list size exceeds the recommended number of patients. Therefore we may not be able to routinely offer you registration and may need to assess the current list size first.

As your personal medical records can take some weeks to be transferred from your previous G.P, we strongly recommend that all new patients undertake a health check with our Practice Nurse to carry out routine recordings such as height, weight & blood pressure and to ascertain your past medical history prior to receiving your medical records.

Violent/Abusive behaviour:

Such behavior, whether towards doctors, staff or other patients, will not be tolerated and will result in a police report and immediate removal from the practice list.

Change of Contact/Personal Details:

If you change your name, address or telephone number please let our receptionists know so that your medical records can be amended appropriately.

If you move address outside our practice area, you will need to find an alternative G.P in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card or alternatively you can contact the Health Authority within the area you have moved to and they will assist you in finding an new G.P.
