Our well-trained receptionists deal with telephone calls, enquires, co-ordinate appointments, repeat prescriptions, medical records and medical supplies.  They will help you use the services we offer.


The Practice Nurses - Baderinwa Olusunmade & Mariamma Shaji

Our Practice Nurse is available by appointment.  They hold Well Person, Well Woman, Asthma, Diabetic and Heart Disease Prevention Clinics. Cervical Smears, Immunizations, Contraception and advice on travel, diet, smoking and alcohol.


The District Nurses -

We have a team of District Nurses based at Manor Farm PCC, Manor Farm Road, Huyton, L36 6GA. They provide treatment and advice for housebound patients recently discharged from hospital. They offer help and advice to those suffering from chronic illnesses or age related problems such as incontinence, mental and physical deterioration. Direct telephone number - 0151 290 4990.


The Midwife - Rose Morton

The Midwife holds an Antenatal Clinic every Thursday 10.00 AM -11.30 AM by appointment at the Childrens Centre, Bedford Close, Huyton. Telephone : 0151 443 5301. She also provides home visit to mothers-to-be, mother and baby to attend to their needs.


The Health Visitor - Jane & Julia Bryant

We have a drop in clinic presently held every Wednesday 9.30 AM - 11.00 AM by appointment.  Your can bring your children for vaccinations, development checks, weighing, other advice regarding child welfare and general advice on health matters.  Direct telephone number – 0151 489 2833   &   0151 511 5922


The Community Matron - Claire Brewer

Matron takes care of the health and social needs of elderly patients at home on four or more regular medications. She co-ordinates with the GP and other services in the community to optimize care.


Other Carers Available Through The Practice:

The Macmillan Nurse visits and cares for cancer patients in their homes and gives advice and support to their families.

Social Services help with many social problems.  Their team includes the Home Help Organizer, Social Workers and Occupational Therapists.

Community Psychiatric Nurses help with the care of mentally ill patients.


Special Needs:

Our surgery has access for disabled patients. We have equipped the surgery with access ramp to enter and also suitable toilet facilities.


To Keep You Healthy We Also Provide The Following Services:

Health Promotion Activities - Well person, Well woman, Asthma, Diabetic and Heart disease prevention Clinics. Clinics are by appointment only.

Cervical Smears - By the Practice Nurse, repeated every 3-5 years.

Child health & Immunizations - The Health Visitor holds Wednesday clinics 9.30 am to 11.30 am.

Post Natal Clinic and Baby Clinic - By appointment only.

Both doctors & midwife provide Maternity Medical Services. Thursday 10 am to 12 pm.

Contraception Services - Advice is given by all Doctors, Practice Nurse, Health Visitor to any patient on request.  Please make a routine appointment.

Minor Surgeries & Vasectomy Clinic: Thursday 1:30 pm to 5 pm. Please discuss this with your doctor who will then arrange for you to be given an appointment.

New Registration Check Up.  When you register with the practice you must attend an appointment for a routine health check with our Practice Nurse.  Please bring your medical card to the surgery and any medications you are taking.

Elderly Check. Our Practice Nurse can see patients over 75 years annually for a health check.


Telephone Advice:

The receptionists have instructions not to interrupt the doctors during surgery consultations unless urgent.  Please feel free to leave a message.


Home Visits:

Our doctors can see four patients in the practice in the time it takes to do a single home visit. For this reason, we ask our patients to come to the practice if at all possible.

Home visits are only for house bound elderly patients or for those too ill to attend surgery.  If you feel a home visit is needed please phone between 9.00 AM -10.30 AM. Our practice operates on a telephone triage system regarding requests to be seen the same day/a home visit or the need to discuss urgent matters with the General Practitioner.

When a patient or relative telephones the Surgery the first point of contact is the receptionist. The receptionist makes a note of the call and request in the telephone triage book, which is forwarded, to the doctor on call. That Doctor then “TRIAGES” the call. The doctor in consultation with the patient decides the appropriate course of medical action. This could be just a telephone advice, a home visit or an urgent appointment at the practice where appropriate examination and recommended treatment can be given.


Medical Education:

We are involved in teaching medical students and other healthcare professionals in training and therefore you may be asked whether you have any objections to there being a student present in the room during your consultation with the doctor or nurse. If you prefer to have a private consultation, please make your views clear. This will not affect your assessment and care and you will not be unfairly treated or disadvantaged.


Non-NHS Services:

The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services.


Confidentiality:

Medical Records are a lifelong history of your health care.  The information is stored on both paper and computer and has details of your consultations, illnesses, treatments, tests and prescriptions.  It is kept as a history of your treatment to ensure that you receive the best possible care now and in the future.  You can help to keep it accurate by telling us of any changes in your circumstances such as new address or telephone number.

All NHS staff are trained in information security and confidentiality.  There are strict rules in place to make sure your information is safe.  Everyone working for the NHS has a legal duty to keep information about you confidential.

All patients have a right to view what is kept on their medical records and any such request must be made in writing to the Practice Manager. You are also entitled to receive a copy of the information that you have seen. However, a small charge will be made to cover administration and photocopying costs. Should the G.P decide that permitting you access to your records may cause serious harm to your physical or mental health, then we may only shown you part of your records or decline your request all together.

In some instances, you may be receiving care from other people as well as the NHS. We may need to share some information about you with them, so that we can all work together for your benefit. Anyone who receives confidential information about you is under a legal duty of confidence. Unless there are exceptional circumstances, for example when the health or safety of others is at risk, we will not disclose your information to any third party without your permission. We will only give your relatives, friends and carers information if you want us to.

You may find that your health records need to be shared with an agency such as a solicitor or insurance company; if so we will need written consent from you where you can indicate whether you wish to view the information that’s sent about you beforehand.

If you have any objections at all to us sharing your information with other medical professions e.g. hospital consultants, please let us know.
